Forms

All players under the age of 18 must have a Parental Consent form signed and notorized or their parent or guardian sign it at the field.

If you would like to see an example of our wavier form that must be filled out by everyone at the field you can click on the Exhibit B link to see one. These are for example only.

Children 12 thru 17 must bring the following forms with them when they come to the field.

1) Parental Consent Waiver. (2 pages) This form must be signed by a parent and notarized.

2) Medical Permission Form.

3) Drivers License or picture ID card.

These forms will be kept on record and do not have to be completed every time the child plays.

Exhibit A is a revocation of consent. If you want your child removed from our consent list, sign and mail it to us at the address below.

Exhibit B is for your information. It is the form your child signs when they arrive at the field.
